How to File a Request for an Appointment
The Mortgage Lending Division requests an appointment to
walk into the office to speak with a division employee. Please fill out the form below and the division will contact
you to schedule your appointment to physically come in and talk with a division
employee. Please e-mail the division at mldinfo@mld.nv.gov for
any questions, or if you need to cancel or reschedule your appointment.

The
Division is located inside the Nevada State Business Center. Address 3300 W.
Sahara Ave Ste 285, Las Vegas, NV 89102. State Business Center will be at the top of the staired
entrance.
